Our Privacy Commitment
The Collision Care Clinic is committed to protecting the privacy of your health information. For payment. We may use or disclose your health information as necessary for payment activities, including billing your auto insurance (MedPay, PIP), health insurance, or managing your lien arrangement.
For healthcare operations. We may use or disclose your health information for operational activities such as quality assessment, staff training, and business management.
As required by law. We will disclose your health information when required to do so by federal, state, or local law.
With your authorization. Other uses and disclosures of your health information will be made only with your written authorization. You may revoke an authorization in writing at any time.
Disclosures Related to Your Auto Accident Case
Because CCC specializes in auto accident care, certain disclosures are common:
To your attorney. With your written authorization, we share treatment records with your legal representative to support your case.
To insurance adjusters. With appropriate authorization, we may provide treatment information to insurance adjusters handling your claim.
To other treating providers. When your managing physician coordinates referrals, relevant treatment information is shared with specialist providers as part of your care coordination.
All disclosures are limited to the minimum information necessary for the stated purpose.
Your Rights
You have the following rights regarding your health information:
Right to access. You may request to inspect and obtain a copy of your health information. We will provide copies within 30 days of your request.
Right to amend. If you believe that health information we maintain about you is incorrect or incomplete, you may request an amendment.
Right to an accounting of disclosures. You may request a list of disclosures we have made of your health information, other than those made for treatment, payment, or healthcare operations.
Right to request restrictions. You may request a restriction on certain uses and disclosures of your health information. We are not required to agree to a restriction, but if we do, we will comply with it.
Right to request confidential communications. You may request that we communicate with you about health matters in a certain way or at a certain location.
Right to a paper copy. You have the right to obtain a paper copy of this notice upon request.

- Zero third-party tracking pixels: The Collision Care Clinic website does not use third-party tracking pixels, advertising cookies, or analytics trackers on pages containing health content. We do not sell or share browsing data with advertisers or data brokers.
- Essential cookies only: The website uses only essential cookies required for site functionality and authentication. No behavioral tracking occurs on health education content.
- Form submissions: Information submitted through contact forms (name, phone number, accident date) is treated as protected health information from the moment it is received. This information is used solely for scheduling your appointment and verifying insurance benefits.
